Child’s Birth Certificate

Proof of Residence

(Lease, Utility Bill, etc.)

WA State Immunization Record that shows requirements have been met. Students cannot begin school in the Fall without being up to date with vaccinations/exemptions. For more information about no-cost vaccinations and scheduling for children up to age 18, go to: www.kingcounty.gov/findaclinic
